I can strongly recommend Malbar from www.malbardesign.net (http://www.malbardesign.net) - I've bought nearly a dozen logos now from him. He provides me with a few to select from, then revisions on them. He does everything in vector and sends you the source files.

Here's some stuff he's done for me. Please, if you use him, tell him I referred you, I'd appreciate it :)

Below are just a few of the logos he did for me. They show the initial mockups/ideas. I then choose one and he does any revisions I want on it:

His prices vary depending on what you order, but for the above type, where you get a few to choose from and then revision, that is $100. He has a 3-day turnaround and is extremely prompt with revisions and everything.
